Enrage tooltip indicates you should get the 40% damage boost for 4 swings. Today while testing heroic strike in SM, one of them critted me and I noticed I did not get 4 swings out of it. Here's the screen shot:

In it you can see the crit which activates enrage, then a Heroic Strike with enrage bonus, then a normal hit with enrage bonus, and the next part is unclear because sometimes a fade message appears directly before the attack to which it applies, but judging by the damage, the HS crit looks to have had the enrage bonus.
So we have a clear case where I only got 3 hits out of the enrage. These took place in rapid succession so there is no chance the enrage timer expired.
