proccing very rarely.. I have 3/5 improved poison talent.. and I'm not even getting them once a fight.

Hi Nawtipo,
Thanks for the report.
After taking many poisoned daggers to the face I think I've isolated the problem. It appears that once a target has a poison debuff re-applications of that debuff do not properly show up in the combat log. They're still being applied and they're still resetting the debuff timer but there is no way to see that on the caster's end.
So you're not losing the effect, it's just not notifying you of it.
Anyhow, I'll get it looked at.
