Magtheridon was a Pit Lord who squatted upon a large portion of Outland, tormenting the Draenei there and coaxing more and more demonkind through the destructive portals therein.
The Laughing Skull clan was a clan of Orcs led by Mogor the Ogre around the time that Durotan, Thrall's father, led the Frostwolf clan; they had a reputation among the clans of being treacherous.

Another clan of the Horde around the same time period. In truth, they earned many accolades in the second war and evaded capture by the Alliance, escaping back to their corrupted homeland of Draenor for a time. The Bleeding Hollow clan was led by another familiar name -- Kilrogg Deadeye. It was from the loss of his eye that the clan was named.

:) You're the bomb dizzle, Cayizzle!
Destromath was a Pit Lord of no true note, I believe, and Dethecus was a Dreadlord of the same ignoble fate.
Gorefiend is the surname of Teron Gorefiend, a Death Knight that rode with the Horde during the second war, around the same era as the clans I mentioned earlier.
Skywall is an elemental... "land" of sorts, obviously centered around the element of air.
Suramar was an ancient city of the Night Elves, I think.

Bloodscalp is a Troll tribe that can be found around Stranglethorn Vale, for example. ;)
Perenolde is the surname of the nobles who ruled Alterac. They are still around, but are not quite as noble as they perhaps once were... ;)
Gorgonnash was another Pit Lord who served with the Burning Legion. No one quite as notable as Mannoroth or Magtheridon, however.

Seriously, though, I'm not positive why she appears quite as she does in the game; lore-wise, however, she is indeed a former High Elf, and that hasn't changed.
Maryanne, the pseudolore reason could easily be that they got better at cartography as the years passed.
Also, remember that many cities with the same name were razed early on, and thus may have changed location. In Warcraft I, for example, the Orcs are ordered to destroy the twin cities of Goldshire and Moonbrook.
However, in all honesty it was probably a question of game mechanics. Stratholme would not have been welcome on the Undead newbie's doorstep, for example. ;)
