Art-wise they're very difficult to create properly without some sort of system handling where the ball and chain should swing with any movement or combination of movements. Without such a system, the ball inevitably sticks out like it's on a pole rather than a chain, and it looks awkward.

Q u o t e: Well, couldn't you just .. animate it's swing animation?
And when walking? Running? Jumping and strafing sideways while running, then spinning around in mid-air, then attacking as you come down?
It's more than just the swing animation. ;)

Q u o t e: What about hair? I watch my dwarves hair swing around while running, swimming, jumping..etc..etc
is there any difference in the way its animated?
Those are set animations and much different from the variety of ways a large metal ball on the end of a chain can move.
The chain on the end of Julie's Dagger and the tassel on the Blackhand Doomsaw do obey gravity somewhat, but those are notably easier than a ball and chain. ;)

Q u o t e: I'll buy that, but only if you riddle me this:
My ears are about eight inches long, and they're ostensibly filled with cartilage. When I take so much as a step they're all bobbing and wiggling and spazzing about. Meanwhile I can sprint off the edge of a windswept cliff, land gracelessly in the water, swim across that water and get into a fight on the opposite shore and the unbound hair that reaches from my scalp down to my belt loops never moves at all.
The large variety of oils and plant extracts Night Elves use to maintain their stylish "do"s unfortunately make their hair extremely stiff. The Gnomes have mastered a similar concoction using Black Vitriol and a lot of machine oil. A lot of oil.
Seriously, though, a weapon swing -- wherein the entire point of the weapon is to swing around this metal ball and smash one's enemy with it -- would look very wrong if it was not doing exactly that.
