I fully understand we cannot discuss peoples individual cases because of privacy, also this is not the place to rectify any unjust banning, so please don't post in this thread about such things and it may survive.
Now onto the point in hand...
There is nowhere I can find in the Manual or on Blizzard's own site to explain how WE the players can protect ourselves from any automated ban, I know the EULA tells us what we can't do but what about in game problems we don't see as exploiting.
For example the other night during an instance run one of our players disconnected as per the usual nowadays, later on that evening he goes to sell his junk loot and see 2 identical grey hammers but only remembers looting one, so like any player he shrugs, sells them both and moves on.
Now is this the correct thing to do if WE the players suspect that poor servers are causing duplicate items, or should we report this issue to a GM immediatly to stop any forthcoming ban in the coming months ahead.
Also if a player were to accidently walk into an unfinished area not knowing it was unfished as they don't read every forum and cheat site around.
WE the players really need some guidelines and acknowledgement that sometimes poor servers and bad terrain are not our fault, I actually feel unsafe playing when the servers are playing up in case an item accidently and unknowingly dupes which at the moment is 90% of the time.

I can understand your frustrations and concerns about these matters, but I must emphasize that we cannot and will not handle ban and suspension cases through the forums. As mentioned in previous locked threads about this issue, this is due to Legal and privacy issues.
Posting about these matters on the forums will not be of any help to your case, as there is nothing we can do for you on the forums. If you feel that your account has been banned unjustly, then please reply to the email that has been sent to you about this matter. If you have not received an email about your ban or suspension, then please send an email to WoWaccountReviewEU@blizzard.com

I cannot understand why you appear to think that trying to keep a lid on this subject will just cause people to forget it.
By banning a guildmate, you just killed out guild bank - hardly fair to do it without any explanation, but I guess thats between you and the poor guy whose account you banned.
What worries me, is that apparantly you ban people without even giving them a fair warning first - is that a reasonable way to treat your paying customers ? It sounds a lot like judgment without a trial or even the chance of defense.

Please inform your guildmate that he should send an email to WoWaccountReviewEU@blizzard.com if he believes that his account has been banned unfairly.

I am unable to answer that question and if that unfortunate event should happen and you do get banned, then all I can do is refer you to that very same email address.
If you do find yourself in a situation where you need to send an email to that address, then I all I can do is to suggest you to be as precise and as detailed as possible in your enquires and in your explanation of what may have happened that resulted in a ban.

Again this is a matter where you would have to send an email to WoWaccountReviewEU@blizzard.com in order to get a clarification of what caused the ban. All questions, concerns and requests about account suspensions are to be handled through this email address.
I know that this is not something that you like to hear, but it is all the help that I can give you. We cannot help you with these matters on the forums.

It can unfortunately take some time before a reply is sent, so please try to be patient. The time it takes to send a reply to the email you sent depends on the time it takes to review your ban or suspension case. Furthermore the amount of emails in the email-queue in front of the email you sent will also have an influence.

Blizz, come on. People have been disconnected from servers because of your faulty hardware/software, and when they log back on they have duped items of the last green items they picked up, like your rollback fudged up somehow. And that is players fault, so they get banned for it? /shrug
Just do the right thing, admit you made a mistake and we can move on.
I too have a guildie who's alt was leveling up enchanting by running through lowbie instances.
And now he's banned from aq/bwl/mc, because of something that wasn't his fault.
Again I wish to make it very clear that this is not something that we can or will discuss on these forums due to legal and privacy issues. Furthermore posts and threads of this nature are often presented in a heavily biased way, which tend to mask and blur the actual facts of what really happened which resulted in a ban or suspension. I can inform you that bans and suspensions are not matters that are taken lightly, and a ban or suspension will not be issued if there is no evidence to confirm that an offense did take place. I really do understand your frustrations, but there is nothing I can do to help you so please try to be a bit less confrontational.
Please send an email to WoWaccountReviewEU@blizzard.com if you have any questions, concerns or complaints about your ban or suspension. If you wish to have you case reviewed, please be as precise and as detailed as possible in your enquires and in your explanation and reasoning of what took place that lead to your ban or suspension.

It seems to me that most of the people talking about this are perfectly aware of what got them or their friends banned, but claim that what happened is of no fault of their own. If this also applies to you, then simply write a constructive and detailed email portraying your side of the story and that you will like to have your case reviewed since you believe that you are innocent.
If you have no clue about why you have been banned or suspended but you are sure that you really are innocent and that you have been treated unfairly, then please state this in your email.
Posting about these things on the forums will not help your cases since all ban and suspension matters are handled through the before mentioned email address.

Are there any ongoing investigations or plans on investigating what really happened on April 4th (or early April) with rollbacks and how these may have affected items? Or is April 4th considered an issue of the past and "resolved"?
This is not a question that I can answer, but I can assure you that all such matters are thoroughly investigated to make sure that no mistakes are made and that appropriate action is taken.
